to protecting your privacy online. This Privacy Policy and Agreement
("Privacy Policy") will tell you what information
we collect about you and your use of our website. It will explain
how we protect that information and what choices you have about
how it is used. We urge you to read this Privacy Policy carefully
so that you clearly understand both our commitment to you and
your privacy and how we collect and use any provided information.
By using the ChipOnTheGo® site, you agree to the terms of this
Privacy Policy. In particular, this privacy policy describes:

Secure SSL Certificate & Encryption
Our SSL certificate is a digital certificate that authenticates
the identity of a Web site and encrypts information that is
sent to the server using Secure Sockets Layer (SSL) technology.
Encryption is the process of scrambling data into an undecipherable
format that can only be returned to a readable format with the
proper decryption key.
A certificate serves as an electronic "passport" that
establishes an online entity's credentials when doing business
on the Web. When an Internet user attempts to send confidential
information to a Web server, the user's browser accesses the
server's digital certificate and establishes a secure connection.
An SSL certificate contains the following information:
The certificate holder's name
The certificate's serial number and expiration date
A copy of the certificate holder's public key
The digital signature of the certificate-issuing authority
The actual encryption
strength on a secure connection using a digital certificate
is determined by the level of encryption supported by the user's
browser and the server that the Web site resides on. For example,
the combination of a Firefox® browser and an Apache Web server
normally enables up to 256-bit AES encryption with our SSL certificates.
This means that depending on the Web browser and Web server
that combine to establish the secure connection through one
of our SSL certificates, the encryption strength of the secure
connection may be 40, 56, 128, or 256 bit.
Complete Confidentiality
Under no circumstances will ChipOnTheGo® release any of your
personal information to any person or company unless ordered
by a court of law. We use state-of-the-art technology to ensure
that your information can be stored and retrieved securely and
safely.
Our service is to help consumers store their medical information
in a safe and secure place and our business model is not dependent
on selling any individual's information.

1. Information we collect
There are a number of ways that you may gain access to a ChipOnTheGo®
service, and how you gain access has some bearing on the information
that is collected from you. You may have directly accessed a
tool from www.ChipOnTheGo.com, or you may have accessed it from
a partner site. A partner site is a site hosted by a third party
that provides ChipOnTheGo® services.
When you register for a ChipOnTheGo® service, we DO NOT collect
any personally identifiable information that can be used to
identify you or contact you. We only collect a ChipOnTheGo®
ID (which can be any random group of characters) and a password
assigned by you. In many cases, you are asked to enter your
registration information directly into our forms. Otherwise,
your information may have been entered into our forms had it
already been entered while in the registration process at a
partner site.
The information provided to ChipOnTheGo® by the partner site
is not retained by ChipOnTheGo® until you complete the ChipOnTheGo®
registration by hitting the "Register" button. If
you choose not to complete the ChipOnTheGo® registration process
and exit our site before hitting the "Register" button,
your registration information is discarded. The partner site
will assign you a unique partner-specific identifier that confirms
to ChipOnTheGo® that you are an authorized member of that partner
site. Once a unique partner-specific identifier is linked to
your registration information, this identifier will always be
used whenever you access a ChipOnTheGo® service from that partner
site. If you access ChipOnTheGo® services from multiple partner
sites, you will have a unique partner-specific identifier for
each partner site.
When you create your medical record, in any language, using
ChipOnTheGo®, you enter your pertinent health information. But
since we do not collect your name, address, email address or
any other personally identifiable information, the health details
that you fill out are NOT linked to you and CANNOT be termed
as personal health information.
2. Children
ChipOnTheGo®'s services and tools are intended for adult use
only. A parent or guardian, however, may use ChipOnTheGo®'s
services to establish a medical record for a minor. The parent
or guardian is solely responsible for providing supervision
of the minor's use of ChipOnTheGo® tools. The parent or guardian
assumes full responsibility for ensuring that the authentication
information is kept secure and that the information submitted
is accurate. The parent or guardian also assumes full responsibility
for the interpretation and use of any information or suggestions
provided through ChipOnTheGo® for the minor.
3. Internal protection of your records
We employ the following practices to protect your privacy:
Encryption: All data on the web site is saved in encrypted mode
by using AES Rijndael 128 bit Encoding algorithm. Even if somebody
were to steal the database, it is virtually impossible to define
not only the information, but the actual host of the information.
Our SSL certificate is a digital certificate that authenticates
the identity of a Web site and encrypts information that is
sent to the server using Secure Sockets Layer (SSL) technology.
Using passwords to authenticate users before granting access
to personal information Limiting access to personal information
to authorized users. ChipOnTheGo® lets you have full control
over who has access to your personal health records. For example,
you may permit your relatives or health care professional
to view your records. Only the person who creates a record
can grant such access to other users.
Despite ChipOnTheGo®'s efforts to protect your personal information,
in certain limited circumstances we may be legally compelled
to release your personal information in response to a court
order, search warrant, subpoena or other valid legal action.
Furthermore, there is always some risk that an unauthorized
third party may find a way to thwart our security systems or
that transmissions of your personal information over the Internet
will be intercepted. At ChipOnTheGo®, however, as all of your
records are anonymous and not directly linked with you, we do
not expect any unauthorized disclosure of your private information.
4. Disclosure of your information
Except as set forth in this section or as specifically agreed
to by you, ChipOnTheGo® will not disclose any Personal Information
it gathers from you on our website. We may release Personal
Information to third parties: (1) to comply with valid legal
requirements such as a current law, regulation, search warrant,
subpoena or court order; or (2) in special cases, such as a
physical threat to you or others. In the event that we are legally
compelled to disclose your Personal Information to a third party,
we will notify you unless doing so would violate the law or
court order.
Disclosure to or by co-branded channel Partners
ChipOnTheGo® is a service provider and provides co-branded services
to websites hosted and operated by companies other than ChipOnTheGo®.
You can only access these co-branded services through the Partner
website. The co-branded ChipOnTheGo® pages that you may access
through a Partner website have different registration processes
and opportunities for information collection. Each of these
co-branded ChipOnTheGo® sites has its own privacy policy posted
on that site, which explains what information is disclosed by
ChipOnTheGo® to the Partner and vice versa. If you visit one
of these co-branded ChipOnTheGo® sites, please read the privacy
policy that is posted on that site, as well as the individual
privacy policy of the Partner website.
Disclosure of aggregate information
ChipOnTheGo® may combine, in a non-identifiable format, the
health information you provide with health information from
other users to create "aggregate data" that may be
disclosed to third parties. For example, ChipOnTheGo® may use
information gathered to create a composite profile of all the
users of a particular partner site. Partnering sites may use
this information to understand community needs and to design
appropriate programs and activities on their site. Aggregate
data does not contain any information that could be used to
contact or identify you. ChipOnTheGo® will not disclose your
personal health information to any third party without your
prior authorization.
5. Third Parties
All partner sites that feature ChipOnTheGo® services are required
to post a statement that references ChipOnTheGo® and its services.
ChipOnTheGo® further requires partners to receive authorization
from users before ChipOnTheGo® will permit the partner access
to any personal health information that a user provides to ChipOnTheGo®.
ChipOnTheGo® currently does not accept banner advertisements
on its corporate site (www.ChipOnTheGo.com). However, if you
access a ChipOnTheGo® service/tool through a partner site, the
Web page containing the ChipOnTheGo® service may feature a banner.
This banner is governed by that partner's policies and practices
regarding such advertisements. Partner banner advertisements
do not track your movement while in a ChipOnTheGo® tool and
cannot be used to target advertising to you based on personal
health information you provide to ChipOnTheGo®. ChipOnTheGo®
may have agreements with some partners to share in the revenue
generated from banner advertisements.
Once you click on a link to a third-party Web site, and leave
the ChipOnTheGo® site, that site's privacy policy, rather than
ChipOnTheGo®'s, governs your activities on that site.
6. Cookies
Cookies are small computer files that we transfer to your computer's
hard drive that allow us to know how often someone visits our
site and the activities they conduct while on our site. Written
as a simple text file, a cookie helps your browser remember
specific information that the Web server can later retrieve.
Some examples of ChipOnTheGo®'s use of cookies include:

Your browser software can be set to reject all cookies. Most
browsers offer instructions on how to reset the browser to reject
cookies in the "Help" section of the toolbar. If you
reject our cookie, certain of the functions and conveniences
of our site may not work properly but you do not have to accept
our cookie in order to use our website in a productive manner.
You should review the privacy policy of other sites you visit
or link to from our site to understand how these other sites
use cookies and how they use the information they collect through
the use of cookies on their own sites. We do not control these
third parties' use of cookies, or how they manage the non-personal
information they gather through them.
7. Your privacy choices
Removing the information
ChipOnTheGo® provides users with the following method for removing
their information from our database, thereby opting not to receive
future communications or to no longer receive our service.

2. Log in to your account using your ChipOnTheGo® ID and Password
3. Go to your account control panel
4. Click on 'Delete ChipOnTheGo® Account'
In order to correct or update your information, you have the
following options for changing and modifying information previously
provided:
Limitations to removing your information
The need to back-up our systems to protect information from
inadvertent loss means that a copy of your personal information
may exist in a non-erasable form that will be difficult or impossible
for us to locate. Nevertheless, we promise that upon receiving
your request, all personal information stored in the databases
actively used for research, daily business activities, and other
readily searchable media, will be deleted.
Changing your information
1.Visit www.ChipOnTheGo.com and login to your account using
your user name and password. You can edit/update your information
at any time.
2. If for some reason you are unable to correct your information,
e-mail us at support@ChipOnTheGo.com
with your request for changes.
Notification of changes in the privacy policy
If we decide to change our privacy policy, we will post these
changes on this page so that you are always aware of what information
we collect, how we use it, and under what circumstances we disclose
it.
